My skin is fair and transluscent. Reviews of some of the fillers state that you can sometimes see the filler that has been injected under the skin. I am interested in a filler for my nasolabial folds. Any suggestions? Thanks. |

Tell them to inject deep. Radiesse (lasts a year) is FDA approved for nasolabials. Silicone(permanent) microinjections is another option. Fat transfer and Restylane type fillers are reported not to last long enough in that area to justify the cost. |

Injecting deep may not be your best answer. If any of the Hyaluronic based filters are injected too superficially, you can end up with a blue discoloration called a Tyndal effect. If Radiesse is injected too superficially you tend to from lumps. Radiesse is intended to be injected under the dermis in the subcutaneous layer or even directly on the periosteum (surface of the bone). If the Hyaluronic Based fillers (Restylane, Perlane, Juvederm, Hyalaform) are injected too deeply, the effect will appear to wear off very quickly.
Radiesse lasts 12-18 months AFTER the second treatment. The actual study that shows that Radiesse will last 12-18 months was done by giving the patients their first treatment at "-4 months" and their second treatment at "0 Months". A singe treatment of Radiesse is not likely going to last 12-18 months...in fact it may only last 3-4 months. Radiesse is 30% powdered synthetic bone (Calcium Hydroxyapetite) and 70% gel carrier. As the gel carrier dissolves, collagen forms around the micro spheres of the powdered bone. After 3-4 months, most of the Gel Carrier may be gone and there may not be enough new collagen to maintain the correction. After the second treatment, you have twice as much powdered bone stimulating the growth of new collagen. At that point you may get a correction that lasts 12-18 months.
Radiesse should not be inected into the lips. It tends to form clumps and lumps that will take a long time to dissolve. Likewise, Radiesse in the tear troughs can look lumpy despite careful injection technique and thorough massaging.
Radiesse is great for the Nasolabial Folds. It comes in 1.3 cc syringes so you get a little more volume than you do with Restylane (1.0 cc) or Juvederm (0.8cc). The one downside of Radiesse is that we don't have anything available to dissolve it if it foms a lump, but that is a very infrequent problem if the injector has good technique. If Restylane or Juvederm makes a lump, they can be dissolved with an injection of Hyaluronidase.
