high absorbtion with patches -- how it works?

You know the nicotine patches that you stick to your skin? Or the hormonal patches women use ...

I wonder how they achieve such a high level of absorbtion that the active ingredient penetrates the skin and enters the blood stream ?

I've read in another discussion here that the same is possible with nano particles but I don't think the patches use such technology ....

I am not sure what vehicle is used in the patch but it could be that by covering the skin the patch creates a moist environment where absorption is enhanced.
