Prevalence of antibiotic-resistant propionibacteria on the skin of acne patients: 10-year surveillance data and snapshot distribution study. [2002.05]
BACKGROUND: Cutaneous propionibacteria are implicated in acne pathogenesis, although their exact role in the genesis of inflammation is still poorly understood. Agents, including antibiotics, that reduce the numbers of propionibacteria on skin are therapeutic. Resistance in the target organism is a well-recognized consequence of antibiotic therapy for acne but formal prevalence and distribution data are lacking. OBJECTIVES: To monitor the prevalence of skin colonization by antibiotic-resistant propionibacteria in acne patients attending the dermatology out-patient clinic at Leeds General Infirmary over a 10-year period beginning in 1991, and to examine the distribution of resistant strains on acne-prone skin and in the nares... CONCLUSIONS: Skin colonization with antibiotic-resistant propionibacteria is much more common now than a decade ago. Resistant propionibacteria are widely distributed on acne-prone skin and in the nares. This suggests that they will be very difficult to eradicate using existing therapeutic regimens, especially from the nasal reservoir.

Photosensitization of DNA damage by glycated proteins. [2002.05]
Photosensitized DNA damage in skin is thought to be an important mechanism of UV phototoxicity. Here we demonstrate that proteins modified by advanced glycation endproducts (AGE-proteins) are photosensitizers of DNA damage and show that multiple mechanisms are involved in AGE-sensitization.Glycated proteins in skin may equally function as potent photosensitizers of DNA damage with implications for photoaging and photocarcinogenesis.

Instrumental evaluation of retinoid-induced skin irritation. [2002.05]
BACKGROUND/PURPOSE: Retinoids like tazarotene are approved for the treatment of chronic plaque psoriasis. In the beginning of topical retinoid therapy, 15-20% of the patients suffer from mild to moderate adverse reactions with burning and erythema. The aim of the study was to find predicative parameters of the individual irritative potential and to suggest options to reduce these initial irritations... CONCLUSION: Adapting retinoid therapy to the patient's skin type can reduce the initial irritative side-effects. During the first days, patients with skin type 1 or 2 should add a medium potency corticosteroid. Stronger skin irritation caused by tazarotene therapy increases therapy effects.

A randomized, parallel, vehicle-controlled comparison of two erythromycin/benzoyl peroxide preparations for acne vulgaris. [2002.05]
BACKGROUND: Topical erythromycin/benzoyl peroxide (EBP), marketed for acne treatment, must be compounded by a pharmacist and requires subsequent refrigeration, warranting the development of alternate formulations. OBJECTIVE: This trial compared the efficacy and tolerability of a single-use EBP combination package (EBP Pak) with those of its matching vehicle control (VC Pak) and the original, reconstituted formulation packaged in a jar (EBP Jar). The matching VC for the original formulation (VC Jar) was used to achieve study blinding... CONCLUSIONS: Results of this 8-week trial demonstrate that the single-use combination package of EBP is well tolerated, effective, and comparable to the original formulation for the treatment of acne vulgaris in this selected patient population.

Valacyclovir prophylaxis for herpes simplex virus infection or infection recurrence following laser skin resurfacing. [2002.04]
BACKGROUND: The CO2 laser for cutaneous resurfacing has been associated with the reactivation of herpes simplex virus (HSV), causing delayed reepithelialization and scarring. Antiviral agents appear to be effective in reducing reactivation, however, the optimal therapeutic regimen has yet to be clearly defined. OBJECTIVE: To assess the reactivation rates of HSV after CO2 laser resurfacing in patients who received prophylactic valacyclovir for either 10 or 14 days... CONCLUSION: These results support the use of valacyclovir in a 10- or 14-day regimen as a preventive agent against HSV outbreaks following facial laser resurfacing.

The effect of a mucopolysaccharide-cartilage complex healing ointment on Er:YAG laser resurfaced facial skin. [2002.04]
BACKGROUND: There are several types of dressings which may be utilized after facial laser resurfacing. Laser surgeons favoring the open type of dressing have used the Aquaphor original formula to reduce the loss of moisture from laser resurfaced skin. The objective of this study was to compare the effectiveness of a mucopolysaccharide-cartilage complex (MCC) containing healing ointment with the standard therapy of Aquaphor ointment in treating erythema, edema, and skin erosion caused by Er:YAG laser resurfacing of facial skin. OBJECTIVE: To compare healing time after Er:YAG laser resurfacing treated side-by-side with MCC as compared with Aquaphor... CONCLUSION: The results of our study demonstrate that MCC healing ointment may provide an advantage over the standard therapy of Aquaphor ointment in the treatment of edema, erosion, and erythema caused by laser resurfacing of facial skin. Mean daily severity scores were consistently lower and improvement tended to occur earlier with MCC healing ointment.

CONCLUSION: The 1.45 microm diode laser is capable of targeting dermal collagen and stimulating fibrosis at depths where solar elastosis resides. Longer unopposed heating times corresponded to increased erythema, edema, and pain, which were typically mild and short-lived. Epidermal burns can result in pitted scars.

Botulinum B treatment of the glabellar and frontalis regions: a dose response analysis. [2002.03]
CONCLUSION: Botulinum B toxin injections represent an alternative to botulinum A treatment for glabellar and frontalis hyperkinetic wrinkles. At the doses utilized in this study, the effect does not generally appear to last as long as has been reported with botulinum A injections. However, the onset of action may be sooner with botulinum B injections.

A side-by-side comparative study of 1064 nm Nd:YAG, 810 nm diode and 755 nm alexandrite lasers for treatment of 0.3-3 mm leg veins. [2002.03]
BACKGROUND: Laser and intense pulsed light device treatments of leg veins have generally yielded disappointing results. Use of longer wavelengths, longer pulse widths, and better cooling devices have recently sparked renewed interests in these methods. OBJECTIVE: To prospectively compare, side by side, a 3-msec cryogen spray-equipped 755 nm alexandrite, a sapphire window cooled super-long-pulse 810 nm diode, and a variable pulse width, cryogen spray-equipped 1064 nm Nd:YAG laser for the treatment of 0.3-3mm leg veins... CONCLUSION: The cryogen spray-equipped 1064 nm Nd:YAG laser was remarkably effective and safe for the treatment of 0.3-3 mm leg veins. The use of topical anesthesia may be needed for some patients. The super-long-pulse 810 nm diode laser gave unpredictable results. Additional refinements of fluence and pulse width could improve its performance. The 3-msec, 755 nm alexandrite laser at fluences of 60-70 J/cm2 and an 8 mm spot can be effective, but inflammatory response, purpura, and matting limit its usefulness. Longer pulse widths might decrease these problems. For leg vein treatment, the 1064 nm wavelength is very safe for type V skin, the 810 nm wavelength at super-long pulse widths of 400-1000 msec is very safe for type IV and marginal for type V skin, and the 755 nm wavelength is limited to nontanned type I-III skin.

Skin irritation typing and grading based on laser Doppler perfusion imaging. [2002.02]

A multicenter, double-blind, randomized comparison study of the efficacy and tolerability of once-daily tazarotene 0.1% gel and adapalene 0.1% gel for the treatment of facial acne vulgaris. [2002.02]
The efficacy and tolerability of tazarotene 0.1% gel and adapalene 0.1% gel were compared in a multicenter, double-blind, randomized, parallel-group study in 145 patients with mild-to-moderate facial acne vulgaris. Both treatments were applied once daily in the evenings for up to 12 weeks... In conclusion, tazarotene 0.1% gel was more effective than adapalene 0.1% gel and was also a more cost-effective treatment option.

UVB photoprotection with antioxidants: effects of oral therapy with d-alpha-tocopherol and ascorbic acid on the minimal erythema dose. [2002.01]

In this study, we performed full-face photorejuvenation using a new IPL device incorporating a 560 nm filter and integrated contact cooling system for the improvement of various symptoms associated with photoaging in Asian patients... CONCLUSION: Full-face photorejuvenation for Asian patients using this device is not only effective but is also associated with fewer post-treatment complications than other more invasive modalities. Combined cervicofacial rhytidectomy and laser skin resurfacing. [2002.01]
PURPOSE: To determine the safety and efficacy of simultaneous cervicofacial rhytidectomy (face lift) and laser skin resurfacing... CONCLUSIONS: Carbon dioxide laser skin resurfacing combined with face lift procedures can achieve dramatic cervicofacial rejuvenation. Laser resurfacing over nonundermined skin during the face lift procedure does not appear to increase the risk of postoperative complication. Laser treatment over an undermined distal face lift flap appears to increase the potential for skin necrosis, and should not be considered a routine modality for facial rejuvenation.
